Learner's Permit Forms

Teens wishing to obtain a learner's permit before their 16th birthday are required to submit a form, #DL-300 Verification of Identity and/or School Attendance, to the Motor Vehicle Administration. Kimberly O'Connor is the contact person who will sign this form for all eligible homeschooled students registered in Baltimore County, including those students who are enrolled under umbrellas. Once a student has reached his/her 16th birthday, this form is not required by the MVA. 
 
A Note from Mrs. O'Connor:
Obtaining a learner's permit is an exciting milestone in a teen's life. Please know that I make every effort to sign and return learner's permit forms in a timely manner. However, my professional responsibilities often occur outside of my office in Towson.  Therefore, I do not advise families to visit the office. Please read the following procedures for obtaining my signature on the required form to ensure a smooth process. COMPLETING THE FORM
1. Print the Learner's Permit Form
2. Complete the section of the form entitled, Section A: Applicant
3. Be sure that the parent/guardian and student have signed the form

SENDING THE FORM 
Send the form (with Section A completed and signed) to Kimberly O'Connor for her signature by e-mail or U.S.P.S. at least 15 days before the student's MVA appointment.

RECEIVING THE FORM
Once Mrs. O'Connor completes the form as specified by the MVA, she will seal the form in an envelope to be opened by the MVA (marked accordingly) and mail the sealed envelope to the parent/guardian in a larger envelope via U.S.P.S. If you wish to pick up the completed form from the receptionist on the 3rd floor of the Towson office (refer to mailing address), please notify Mrs. O'Connor when you send the form.
